The Website may offer forms for submitting inquiries, scheduling consultations, or initiating services. Submission of any form, inquiry, or communication through the Website or via email does not constitute a formal agreement or commitment by us to provide services and does not create a client-service provider relationship. We are not required to provide services unless a separate agreement and any required consent or other applicable forms are completed. 

Marketing and Communication

By providing your contact information through the Website and forms, you consent to receiving communications from us. These communications may include: (1) outreach via telephone and/or email regarding your inquiries or potential services; and (2) marketing communications such as emails and SMS notifications, should you choose to sign up for these, where permitted by applicable law.

Note that opting out of marketing communications will not affect your receipt of service-related communications or your responsibility for any outstanding balances.

If you make a payment to us on or through our Website, all information you provide during the transaction, such as your name, address, method of payment, credit card number, and billing information, may be collected by both us and our payment processing company. User agrees to use the Website only for lawful purposes. The information on this website was prepared by Clinician On A Mission LLC. It is for informational purposes only, and is not intended to constitute, nor does it constitute, medical advice or treatment for any condition.

As with all forms of exercise and movement, Yoga, Reiki & Sound Healing all carry some level of inherent risk. Clients are responsible for consulting with their medical and mental health providers to ensure there are no contraindications to participation in these services at this time.

Use of this website does not create a therapeutic relationship nor a service relationship. A consultation to ensure that the service provider is the right “fit” for your unique needs is required prior to enrollment in services. Service agreements and informed consent procedures must be formally signed prior to start of services. It is the responsibility of the client to ensure that the service type they seeks is appropriate for their current needs and is approved by a licensed medical provider.

The website is not intended to be used for advice. Individual results are not guaranteed and no claims are made as to symptom improvement or progression. Clients reserve the right to discontinue services, cancel or decline to schedule future service appointments and forfeit remaining sessions they have paid for for any reason at any time. All sales are final and non-transferrable. No refunds will be offered. Clients will be billed the full amount for all completed service appointments and sessions, including sessions terminated early due to inappropriate communication or misconduct. Clinician On A Mission seeks to cultivate a safe & respectful healing environment to facilitate trauma-informed care. Clients are welcome and encouraged to practice self-advocacy to verbally express preferences regarding practitioner’s use of hands-on assistance, diffusing of essential oils, lighting & temperature at any time during their sessions. Clinician On A Mission maintains a Zero Tolerance Policy for sexual harassment, whether verbal or non-verbal. All clients are expected to conduct themselves in appropriate and respectful ways. The practitioner reserves the right to discontinue any session due to inappropriate language and/or behavior. In the event of client misconduct and session termination, all future sessions will be forfeited and no refunds will be provided.

As a mandated reported in Massachusetts, Provider is legally required to notify the authorities if any client expresses intent, means and access to engage in self-harming behaviors, threatens someone else or in the case of disclosure of suspected abuse of a minor or elderly or disabled person. Provider will inform you if required to file any such claim.
